Vitiligo A depigmenting disease of unknown origin that causes destruction of melanocytes, vitiligo is estimated to affect at least 1 percent of the population, and one study found that 50 percent of cases begin before the age of 20.6 Some studies suggest 30 percent of cases are genetic.7 There are two main types of vitiligo: nonsegmental (also referred to as generalized vitiligo) and segmental (also referred to as localized vitiligo). Approximately 90 percent of those who have vitiligo have the nonsegmental type.8 Found symmetrically on both sides of the body, nonsegmental vitiligo presents as depigmented macules and patches described by size around 0.5–5 centimeters with well-defined borders. The borders may have a red halo of inflammation or rim of hyperpigmentation. It may be active and progressive at times and go into remission. It is responsive to medical therapies, especially when started early. This is why being an astute skin care professional is so valuable. Common to all types of vitiligo, loss of pigmentation is not as obvious in pale skin, but it's more obvious as the contrast between skin colors deepens. This type of vitiligo also has a predilection for orifices, including the eyes, nostrils, mouth, nipples, umbilicus, and anus. Whitening of beard hair, eyelashes, and eyebrows also occurs. People with vitiligo commonly report that their first onset came in the aftermath of emotional stress, illness, or skin trauma such as sunburn or a bruise (Koebner's phenomenon).
